 Yes I was declined.  Sorry, I assumed you would have a record you could examine.  I have to say, it is demeaning to be accused of supplying false data, since filling out only four fields cannot possibly be construed as "incomplete".  Perhaps you need to give us a hint about what you are looking for.

 There are only four boxes to fill out, and I promise I filled them all out accurately.  Who is reviewing at 10:00 Friday night?  Please don't tell me this is a bot searching public records or some silliness *smile*.  Let me know what I need to do, and I will gladly jump through whatever hoops required.
