Thanks, if you can get more error urls and the friend IDs of the folks who are having the problem, it would be helpful. :)

I'm able to reproduce this on different apps. Don't think it is user or app related.

The app is reported as installed and redirected to the canvas after clicking on "Add" button but actually is not installed. I think we loose 10-15% of installs here. This is critical.

 Will this ever get fixed?
